We are excited that you are interested in the process of joining a mission team with Ebenezer Baptist Church! It is our prayer that you will hear God’s calling for you to help carry out our mission of being an urban based, global ministry dedicated to individual growth and social transformation.

Participating in a mission trip will have a profound impact on your life, whether this is your first or one of many that you have done in the past. God will cultivate you and transform you in a special way through your mission experience. T

he attached application is an essential part of the trip process. Please read the contents of this application and be prepared to return the application according to your mission trip application deadline.

INSTRUCTIONS FOR COMPLETING THE MISSION TRIP APPLICATION

● Ebenezer Baptist Church uses this application for all mission trips. Once it is completed, it is maintained on file with Ebenezer Baptist Church. Even if you have filled out an application for a previous mission trip, the entire application will need to be completed for each new mission trip.

● Please fill out completely.

● Do not omit any pages from the information form.

● A photocopy of your passport will need to be provided to the partnering Travel Agency when purchasing plane tickets and booking international hotels.

● Make sure your passport is signed and that the expiration date is more than six (6) months subsequent to the trip dates.

By signing below, the Team Member understands that any required deposit is subject to the partnering Travel Agency’s terms and conditions including their refund policy or cancellation policy and he/she will be responsible for airline tickets purchased in his/her names. The monthly meetings for this mission project are critical for the spiritual unity and physical preparation of the entire team. The Team Member commits to faithfully attend all meetings at the scheduled times.

LIABILITY ACKNOWLEDGEMENT

I, (your name here) in consideration of my acceptance (Team Member) as a Team Member on a short-term mission trip sponsored by Ebenezer Baptist Church to represent that: (Destination of Trip)

1. I am aware of the potential hazards and risks to my person and property associated with serving in a mission’s capacity, such hazards and risks including, but not being limited to, injury or death by accident or intent, disease, war, terrorist acts, weather conditions, inadequate medical services and supplies, and criminal activity and acts. I choose to go on this trip with full awareness of these risks and I will rely upon my personal insurance coverage for anything the travel insurance does not cover. I accept these risks, recognizing that Ebenezer Baptist Church would not be able to offer the opportunity for missions’ service without a release such as this. With respect to Ebenezer Baptist Church and its agents, officers, volunteers, directors, and employees, I voluntarily assume all risks of death, injury, illness or loss associated with such risks, and any damage to my personal property, and I release Ebenezer Baptist Church and its agents, officers, directors, and employees from any liability that I may suffer or claims I may have as a result of participation in a mission trip even if resulting from the negligence of Ebenezer Baptist Church, its agents, officers, volunteers, directors, and employees. I further recognize that such risks have always been associated with mission trips.

2. I attest and certify that I have no medical conditions that would prevent me from performing my duties.

3. I give permission to be photographed, and/or recorded, with video and/or sound, for promoting Ebenezer Baptist Church and its activities, and/or for creating an audio or video product of the trip for sale, promotional, or marketing purposes.

4. I understand that travel insurance may or may not be provided for the trip in which I am participating, and it is my responsibility to determine if travel insurance is provided.

5. I expressly agree that this Liability Acknowledgement is intended to be as broad and as inclusive as permitted by law. I further state that I have carefully read the foregoing and understand its contents, and I voluntarily sign this Liability Acknowledgement as my own free act. I also understand that this is a legal document and I have the right to consult with an attorney before signing it.

MISSION TRIP POLICIES

Ebenezer Baptist Church

● Financial Policy for Team Members: Team Members are responsible for funding 100% of their expenses for the trip. Funding will be required to be completed by the due date(s) set by the Global Missions Strategy Team. No tickets will be purchased until the required funds have been received.

● Policies for Team Member Behavior and Attitude: Team Members are reminded that they are ambassadors of Ebenezer Baptist Church. As Team Members travel overseas, they not only represent Christ but also Ebenezer Baptist Church, our Pastor, the United States and the non-profit or church we are assisting. This is a tremendous responsibility. For this reason, Ebenezer Baptist Church asks that each Team Member seek to be above reproach in his/her actions and attitudes.

● Team Members must submit to the Team Leader’s authority.

● Due to the political instability and anti-American sentiment in various countries around the world, Ebenezer Baptist Church asks that Team Members refrain from expressing political opinions while overseas.

● All Team Members must abstain from the consumption of illegal drugs.

● Each Team Member must adhere to the behavioral guidelines for the team set by the Team Leader and/or the non-profit and/or church we are assisting with a mind toward the culture of the country to which the team is going. This will require a servant attitude toward all nationals and other Team Members, as well as the willingness to learn from the host culture.
